Population Health Management

Healthcare delivery is increasingly driven by data, shaping decisions across healthcare organizations and systems. As the industry evolves, healthcare managers are expected to apply population health management strategies that improve access and quality of care while managing costs.

Seton Hall’s 15-credit Graduate Certificate in Population Health Management provides a comprehensive overview of population health approaches, strategies and tactics used to support better outcomes across diverse care settings. Offered fully online and taught by the same faculty as the University’s CAHME-accredited Master of Healthcare Administration (MHA) program, the certificate emphasizes practical applications in data analytics and patient engagement, preparing professionals to implement population health initiatives and support a culture of wellness.

Joe Conte

"We build relationships with community-based organizations. We do hot-spotting and geo-mapping to understand the prevalence of specific conditions to assess the need for program development. For example, on Staten Island, we put together a program where patients with substance use disorder can access a provider 24/7 and talk to a peer-counselor who has been in their shoes, and we partner with the DA’s office on recovery programs."

Joe Conte, MPA, CPHQ, PhD ’17
Seton Hall Alumnus and Executive Director of the Staten Island Performing Provider System, LLC
